Download and print the forms listed here. The forms are in pdf format. If you are not able to open the form by clicking on it, try right-clicking on the icon or name of the form. After you right click, select the option that says "Save as Target" or "Save Link as." Use the dialogue box that appears next to save the form to your computer.
You have the following options for submitting the form:

- You can mail it to us.
- You can email it to us.
To email it to us, you must print out the document and scan it by hand. After you sign your own print out with a handwritten signature, you can email us the image of the scan as an attachment. Your scanned image must be in either pdf, jpg, or png format. It must also be a resolution of at least 200dpi with margins at 8.5W x 11H. If your scan does not meet this criteria, we will not accept it. No exceptions. Email the scan to:
